Introduction for New Patients

If you haven't already done so, we recommend you look through the section About Us to find everything you need about Steeplechase Veterinary Hospital and how we can help you.    Here we provide you with information that should be helpful before your first visit.  There is information on what to expect, to remove any mystery about what will happen when you make your first visit to our Cumming veterinarian clinic.  There are online new client forms if you would like to complete one in the comfort of your home or office:

• New Medical Client Form if you are to see Dr. Amiri for medical services
• Grooming Client Form if you expect to see only Jo or Jack for grooming services
• New Behavioral Client Form if you are havng your first appointment with  Dr. Owens for a behavioral consult
